Individual Nutrition Counseling
A Registered Dietitian will provide personalized diet instruction, menu planning, lifestyle and behavioral changes to insure a healthy nutrition plan.
Scheduling an Appointment
Appointments for nutrition services can be made by calling 773.296.7018. A written referral from your physician which includes a diagnosis and type of nutrition counseling desired is required at the time that you make your appointment. Also please bring your insurance card on the day of your appointment.
