Your surgical experience at Lutheran General Hospital starts after we get a call from your surgeon's scheduler. To start the process you will receive a call from Patient Registration to be pre-registered by one of our clerks, who will request information including insurance information. Pre-Registration is very important and we would appreciate you returning any phone messages requesting pre-registration. Your insurer can verify whether Advocate Lutheran Hospital is included in your network, or you can view participating insurance plans here. You should always check with your insurance provider to verify coverage for your specific procedure. You may also contact our registration office to pre-register or ask questions about insurance at 847-723-1900.
In addition, some care providers who may be part of the surgery team are not employed by Advocate, such as anesthesiologists, and may have separate contracts with managed care plans. Your coverage for these services may be different than the coverage for the hospital. Please check with your insurance carrier.
The next step in preparing for your surgery will be to complete a health history. You may be scheduled for a phone appointment with the Pre Surgical Testing Department. On the day of your phone appointment, the pre-surgical testing (PST) nurse will ask for key information, including:
Your medication list, complete with dosages and the times you take them.
Your history of past surgeries and medical conditions.
The names and phone numbers of your doctors.
The name and phone number of your pharmacy.
The nurse will use this information, along with orders from your doctor, to determine what testing is needed prior to surgery. The PST nurse is in communication with your surgeon's office staff, and will evaluate your tests and answer any questions you may have. The PST nurse will gather all pertinent information and complete your chart prior to the day of surgery.
The decision on where to complete any testing will be decided based on your insurance requirements, the surgeon, your primary care/family physician, and/or your preference. You may have the option to choose Lutheran General Hospital’s Center for Advanced Care, located on the hospital campus at 1700 Luther Lane. Map and directions
The nurse may review your test results or other medical issues with a physician, to determine your readiness for your procedure. If you are scheduled for a procedure at Lutheran General Hospital, you should review our preoperative instructions. In addition, the Pre Surgical Testing Nurse will review information tailored to your unique situation, including medication instructions.
You will be asked whether you have current advanced directives. These forms are not mandatory, but advisable for adults to have thoughtfully completed. To download the forms click below:
