Financial assistance offers free or low-cost health care, depending on your eligibility. No extra steps are needed for qualifying patients. Our financial counselors will automatically review your eligibility based on your income. 

Common questions about Neighborhood Care

No appointment is needed at our health clinic. Simply walk in for same-day services. You can also call us at 312-268-6627 to schedule an appointment that works for you at your preferred Neighborhood Care location. 

You’ll need a valid photo ID for your appointment. If you’re insured, please bring your insurance card and a form of payment, in case this is due at the time of service. We accept most major insurance plans as well as credit cards and debit cards. 

We care for the uninsured, underinsured and insured. We offer income-based financial assistance, and our financial counselors will work with you to determine your eligibility. 

At our community clinic, we combine an in-person experience with virtual care technology. Our in-person care team assists with your exam while you interact with a virtual family medicine clinician over video.  

This advanced technology enables us to assess your ears, nose, throat, heart, lungs and skin. During your exam, you’ll see the same image the doctor sees. This innovative care model ensures you receive quality, expert care.

We’re here to make care easy – especially when you need it but can’t see your primary care provider right away. If you don’t have a primary care provider, we can help you find a provider for ongoing care, preventative health and managing chronic conditions – just let us know during your visit.
