Advocate Health Care’s palliative care team layers support to help you and your family manage your symptoms, reduce the number of hospital or emergency department visits and determine goals for care.
When do you need palliative care?
You may benefit from Advocate Health Care’s Palliative Care Program after you've been diagnosed with a chronic or serious illness, including:
- Alzheimer’s and dementia
- Amyotrophic lateral sclerosis (ALS)
- Cancer
- Chronic obstructive pulmonary disease (COPD)
- Heart failure
- Kidney failure
- Liver disease
- Multiple sclerosis (MS)
- Stroke
Our palliative care team is here for you
Your palliative care team may consist of a board-certified palliative medicine physician, an advanced practice clinician, a registered nurse, a social worker and a chaplain. They will work with you and your family to plan goals for care, help manage pain and help you understand treatment options.
Our team will:
- Help you navigate a complex health care system
- Help with relief from your symptoms, including pain, stress, shortness of breath and nausea
- Work with and facilitate communication with your care team
- Connect you to other resources
- Answer your questions about your illness and care options
- Help create a care plan based on your individual needs and goals
- Listen to your concerns and help ease your anxiety and worry about your illness

The difference between palliative care and hospice care
