Here's what you and your loved ones need to know when it's time to deliver your baby at Advocate Good Samaritan Hospital.

Patient admitting process & guidelines

Patients will be admitted directly to the labor and delivery unit. During the admission process, patients must sign a health care consent form and provide their demographic information. Patients can choose to submit this information ahead of time by using our online OB pre-registration form. The form covers consent to treat, responsibility for payment, assignment of benefits, release of medical information, independent physician services and declares that the hospital is not responsible for a patient's valuables.

If you have not already done so, check with your place of employment or your insurance company to confirm the type of coverage that your insurance policy provides or to determine any unique requirements in your policy such as pre-admission certification or required notifications. All non-covered charges are payable when you are discharged.

What to bring with you

  • Insurance card, referral or pre-certification form or number.
  • Any written orders or paperwork from your physician.
  • Name of pediatrician. To find one, call 800-3-ADVOCATE or search online now.
  • Toiletries and other personal items.
