The Diabetes & Nutrition Care Center at Advocate Sherman Hospital gives you and your family the information and skills necessary to successfully manage your diabetes. Using safe and proven methods, we'll tailor a plan to your specific health needs.
Our Diabetes & Nutrition Care Center offers:
- Expert staff (English and Spanish speakers)
- Diabetes education and weight management
- Nutrition and diet coaching
- Medication financial assistance (for those who qualify)
The American Diabetes Association recognizes Advocate Sherman Hospital's diabetes education program for consistently meeting the national standards for self-management education.

Team approach to diabetes care
Our experienced team of physicians and clinical educators provides advice and support to help you and your family understand and manage your symptoms and stay on track with your health. Our team includes English and Spanish speakers, with access to interpreters for a variety of other languages as well.
Primary care physician: Your primary care doctor, who may be a primary care or family practice physician, is who you would see for general checkups and when you become ill. Find a doctor.
Certified diabetes care and education specialist (CDCES) dietitian / nurse: A CDCES is a dietitian or registered nurse educator who has received special training in caring for and educating diabetics and national certification. CDCESs help you and your family learn the day-to-day aspects of diabetes self-management. They also provide your referring physician with periodic written updates on your progress.

- Heart and vascular care through our Advocate Heart Institute
Personalized diabetes education
Being informed is the first step to taking control of your health and not fearing the unknown. You and your family are not alone in coming up with a plan of treatment. Your certified diabetes care specialist will teach you and your family about:
- Diabetic nutrition and exercise
- Foot and skin care
- Glucose self-monitoring
- Medical regimen and lifestyle
- Insulin dosing, timing and self-administration instruction
- Additional resources such as group counseling
Nutrition coaching
We will connect you with a registered dietitian who can help you with a custom meal plan for:
- Healthy eating
- Weight loss
- Disease management
- Heart health
- Gluten-free
- Low sodium
- Food allergies
