Diabetes & endocrinology

Advocate Condell Medical Center is committed to helping patients manage their diabetes and avoid any related complications. Our Diabetes Self-Management Program is recognized by the American Diabetes Association and customized for you and your specific needs and goals. It combines diabetes education, nutrition education and counseling, fitness expertise, continuing education classes and support groups to help you control and maintain your blood glucose levels and associated diabetes symptoms.

Diabetes program features

• Assessment of each patient's education needs.
• Series of three classes taught over a one month period led by a certified diabetes nurse educator or a dietitian.
• Dietary needs assessment and development of an individualized food plan.
• Individualized action plan with specific goals.
• Periodic assessment of goal achievement.
• One-on-one education available with a diabetes educator and nutritionist.
• Refresher class offered yearly thereafter to provide follow-up diabetes self- management education and support.
• Specialized education program tailored for patients with gestational diabetes available.

Insurance & payment information

Because our diabetes program is ADA certified, most insurances and Medicare will cover diabetes education. Please contact your insurance company to determine your coverage and if a co-pay is required. A physician's order is necessary to bill your insurance. If you have an HMO, you'll need a referral from your primary care physician.

Diabetes support group

This free support group is open to adults with diabetes and their family and friends. It meets every month at Condell Medical Center. Call 800-3-ADVOCATE for more information on location and meeting topics.
